Geriatric Assessments and Referrals

A geriatric assessment provides an in-depth look at the older individual’s living situation, functional ability, and overall state of well-being. Through Carative’s comprehensive assessments our nurses are able to gain valuable information about an individual’s health and their cognitive, psychological, social, legal and financial status. With this knowledge we are able to formulate an individualized care plan and make recommendations and referrals accordingly. Assessment and referral services we provide include:

Perform geriatric assessments to identify problems and provide solutions and referrals
Evaluate long term residential care choices and assist with moving an older adult
Arrange for driving evaluations by professionals specializing in older driver safety assessments
Record the health history
